Why would a Russian spy talk on a cellphone, While driving a CAR? What a stupid way to attract the police!

Hell, it would just make him look like every other American.

The real stupidity in the episode was how ridiculously easy it was for a
GRU colonel to access the most important protected witness in the US
government and kill him.

That whole hospital wing would have been in lockdown, every door guarded
by marshals, especially the door to his room. All staff that had access
to the patient would have been vetted with a full background and known
by sight to the marshals. A stranger in a set of hospital scrubs with a
stolen keycard wouldn't have cut it.

This was a much bigger plot hole than not using the bluetooth in the car.

The superhuman ability to find witnesses is a hallmark of spies on movies and TV.

The distracted single guard is typical though, isn't it?
